Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
• Do I have mental health benefits?
• What is my deductible and has it been met?
• How many sessions per calendar year does my plan cover?
• How much does my plan cover for an out-of-network provider?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

I’m currently a network provider for the following insurance carriers:
Aetna, Horizon BCBS
Ceridian EAP

If you don’t see your insurance provider on the list below, please contact me as I may be covered in their network or already have an out of network provider agreement with your carrier.

Reduced Fee and Self-pay
If you do not have insurance or wish to pay out of pocket for privacy purposes, please contact me for to find out more information.

Payment
Cash, check, and major credit cards are acceptable.

Cancellation Policy
I understand that unforeseen circumstances may cause you to reschedule or cancel an appointment. In the event that you need to cancel an appointment, please let me know as far in advance as possible, but at least 24 hours ahead. You will be charged $75 for any appointment not canceled 24 hours in advance. I will waive this fee if we are able to make up the missed appointment that same week. Please note that insurance carriers do not reimburse for missed appointments.
